POSITION SUMMARY

 

The Health Records Clerk is responsible for clerical duties including all aspects of patient chart maintenance.

 

RESPONSIBILITIES:

 

Processing Admissions & Discharges:

 

  • Locate and deliver patient charts to inpatient unit as per daily admission register
  • Process Inpatient Discharges, including assembly of records, scanning records to eChart/EMR, analyzing records for completeness, issuing Incomplete Record (ICR) deficiencies to physicians and abstracting the data into Med2020 WinRecs. 

 

Processing Outpatient Records:

 

  • Analyze records, identify deficiencies and issue ICR deficiency to physicians
  • Scan records to eChart/EMR
  • Finalize ICR process once physicians complete deficiencies

 

Other Departmental Duties:

 

  • Perform Meditech Expanse chart maintenance as required
  • Assist in the preparation of records for Death Chart Review
  • Assist with month-end statistics
  • Assist with monthly CIHI and mandatory quarterly Ministry of Health submissions
  • Attend monthly ONE team meetings (as applicable)
  • Sort, compile, index and file health records

 

Day Surgery/Specialist Clinics:

 

  • Locate and deliver charts for Day Surgery and Specialists Clinics
  • Scan records to eChart/EMR

 

Reception:

 

  • Perform department reception duties
  • Respond to clinician requests
  • Respond to internal and external general requests

 

Record Retention:

 

  • Review, record and prepare appropriate charts for destruction

 

Admitting:

 

  • Assist with switchboard, reception, patient registration and other admitting duties as required, to cover breaks and/or backfill shifts as required.

  • Use and wear equipment and protective devices that are provided
  • Report to the supervisor absent or defective equipment or protective devices
  • Report the existence of hazards to the supervisor
  • Not remove or make ineffective any protective devices without providing an adequate temporary protective device
  • Operate equipment in a safe manner
  • Not engage in horseplay
  • Be familiar with procedures to be carried out in the event of a disaster (Emergency Preparedness Plans)

COMPETENCIES/TECHNICAL:

 

  • Proficiency in computer software applications: Microsoft Office, Excel, Meditech ADM (Admissions), Meditech MRI (Medical Records), Meditech SCA (Scanning/Archiving), EMR; Policy & Procedures platform and Learning Management System
  • Typing speed minimum 50wpm
  • Knowledge of PHIPA (Personal Health Information Protection Act)
  • Knowledge of Public Hospitals Act
  • Proficiency in Med2020 WinRecs abstracting standards


QUALIFICATIONS (Education, Certifications, Experience, Licensure)

 

Required:

 

  • Secondary school diploma
  • Mandatory proof of fully immunized COVID-19 vaccination status

 

Preferred:

 

  • Certificate in medical terminology from a recognized college or equivalent
  • Medical transcription training/experience
  • Experience in a healthcare office/clerical environment

 

OTHER QUALIFICATIONS:

 

  • Ability to provide regular attendance.

 

WAGES AND BENEFITS:

 

  • Starting wage of $25.01
  • Enrollment in HOOPP (Health of Ontario Pension Plan)
